“My Day, My Way” is a project to help us learn more about what life is like for local people with a long-term conditions and disabilities.
We want to know:

- What things are hard for you and your family?
- What do you want for your loved one’s future?
- What kind of help or support works best?
Why are we doing this?
We believe everyone should have the chance to live a life that feels good, safe, and full of choice.
We want to hear from you about:
- What you want daily life to look like for your loved one in the future
- What helps you and your loved one to feel part of your community
- What stops you and your loved one from joining in with local activities
- What support you need to feel happy, safe, and included
- What helps your loved one to learn, work, or do things they enjoy
