Chaperone application form

To apply for a Chaperone Licence from Wigan Council you must be a local resident and pay your Council Tax to Wigan Council.

Before completing this form please review the Chaperone approval licences web page for detailed instructions.

You must have the following documents ready to upload before you start the chaperone application process.  There is no option to save and return to the application later.

  • Passport style photo (a selfie is fine)
  • Safeguarding Children Level 2 or higher training (volunteer applicants only)
  • NSPCC Protecting children in entertainment training for chaperones (professional applicants-compulsory, volunteer applicants-recommended)
  • Letter from Principal/Manager of the dance school/organisation (volunteer applicants only)
  • DBS ID Verification Form completed by the Principal/Manager of the dance school/organisation (volunteer applicants only)

Please note processing a Chaperone Licence takes a minimum of 12 weeks.

Important information before you start this form:
